2012 “NEW” MEMBERSHIP APPLICATION

For the 2012 Calendar year the PMGA will receive licensing directly with the USGA. Our handicap computation will be provided by the Amateur Association of Arizona Golfers (AAAG), an entity being supported by the Southwest Section of the PGA. You will receive a Handicap Index and be able to posr your scores throughout the United States online and at the golf course facility. Currently this handicap is not recognized locally by the Arizona Golf Association. If you would like to play in an AGA event, you must join the AGA directly or through another club or facility.
FEES INCLUDE FREE ENTRY INTO THE AUGUST HOME EVENT. PLEASE FILL OUT THIS FORM COMPLETELY. AN EMAIL ADDRESS HELPS US COMMUNICATE BETTER WITH YOU. WE DO NOT SHARE THIS INFORMATION!!
Upon acceptance of my application I agree to abide by all the rules and decisions of the PMGA. The PMGA reserves the right to cancel or relocate PMGA events if Papago Golf course becomes unavailable for play. I understand that the Board of Governors has authority under the USGA handicapping system to adjust my handicap. I understand that I must post an individual score in two (2) PMGA regular monthly tournaments since the last Club Championship to be eligible for the 2012 Club Championship. If at any time in the past I have violated the USGA Rules of Amateur Status and have not filed for reinstatement, I will not be eligible to play in the Match Play, Bangkok, or PMGA Club Championship. The Club Champion must be an amateur golfer. I must have at least ten posted scores in the last twelve months to be eligible for PMGA events. I must participate in two (2) PMGA sponsored events within the calendar year to be eligible for membership renewal. Tournament entry fees may be refunded at the discretion of the Board. It is the member’s responsibility to keep contact information up to date. All correspondence and notices will be sent first to members who provide an email address. U.S. mail will be used if an email is not provided. Not all correspondence and notices will be sent by U.S. mail. Contact information is not sold or shared.
